UPDATED SHOW RECORD FORM
Review form. Become familiar with it. It's TWO pages now. Download your show record form and fill it out ahead of time.
It MUST BE filled out COMPLETELY & CORRECTLY and SIGNED by a show officiant. We will do audits and we will question if things look abnormal.
Double check those head count numbers too! Don't go it alone.
Send it in within 30 days of the show. No exceptions. Email or via USPS.

The show record form is your enrollment form. MUST BE COMPLE
TELY FILLED OUT. It will not be accepted and sent back if it is not. Avoid laziness. Avoid shortcuts.
